The installation failed in the SAFE_OS phase with an error during BOOT operations.

Results online indicate you should remove AV software, ensure drivers and firmware are updated, sfc /scannow, etc. Those probably won't fix it, but are good to do.

This will probably fix your issue (this has worked many times for me):
Recreate the EFI partition. You will need to boot from recovery (or Windows Install disk), or put the drive in a different computer so the drive is not active. Delete and recreate the EFI partition.

Another possibility is to rebuild the Windows Recovery partition. This was not necessary for me yet, however it has fixed the issue for some other people.
