Guide Issues Windows Update Windows 10/11 Update/Upgrade Failure Analysis The installation failed in the SAFE_OS phase with an error during BOOT operation - 0x8007042b - 0x2000d Updated February 14 2026
I am getting this on a Win11 25H2 computer which is having issues when trying to update to fix some corrupt files.

The fix was to use Windows 11 Media Creation tool to generate a new Win11 25H2 ISO file which includes monthly security updates to today. I then used Rufus to put it on a USB, inserted it into the computer, and did an in-place upgrade by running setup.exe on the USB. It installed properly.

I believe this happened because the laptop has 25H2 installed with monthly security updates through January 2026, and the Win11 25H2 USB I had originally tried was from when 25H2 came out, months earlier, and so was an earlier build. By making a new USB the build version was now updated to Feb 2026, and so was a later version and it allowed the upgrade.
